YCS Concerns/Suggestions Form

Thank you for contacting the Office of the YCS Ombudsperson. The goal of this office is to address concerns related to the safety, well-being, and rights of all YCS participants and staff.

Please complete the form below with as much detail as possible so that the concern can be reviewed appropriately. You may choose to remain anonymous; however, please note that if a concern is submitted anonymously, the Ombudsperson may not be able to follow up directly with you or provide updates regarding the status or resolution of the concern. Providing a phone number and/or email address allows the Ombudsperson to follow up within 24–48 hours if additional information is needed.

Please Note: The Ombudsperson process is intended for concerns related to safety, ethics, rights, or organizational conduct. Routine operational matters (e.g., maintenance requests, scheduling, supervision questions, etc.) should first be addressed through the appropriate supervisory chain of command so they can be resolved as quickly as possible.

YCS also requires that employees report any incidents that conflict with YCS policy, training, mission statement, a client’s human rights, and/or the law to Human Resources and/or Quality Assurance through established reporting channels.
