Documents on this page are in fillable PDF files and 13 of these forms require signatures. There are three different ways you can sign these documents:

Using Adobe Acrobat Reader DC. Use the Fill & Sign feature which will allow you to draw or write your signature on the document (View Tutorial) or create digital signature (Digital Signature Tutorial).

Using Microsoft Edge. The Microsoft Edge Internet browser has an add notes feature that lets you write using your
mouse on a PDF document. Use this feature to sign any of the Outpatient Consent document (dont forget to save the file after signing).

Download, print, sign and scan. If you have a scanner at home, this is the way to go.
Email the document(s) to institute@ycs.org once signed.
